 Event Chairman's Paperwork 

Event chairmen are required to keep certain records of their event, as well as observe certain protocols. The documents below allow you to easily keep track of money and hours of service. These forms were created by our Lion Secretary to help you, they are also required to plan and complete an event.

Event Guidelines

Use this to help plan your event. The guideline lists your responsibilities, explains what needs to be done and who to talk to for assistance on licensing, permits, insurance, operating cash, chairing events and covers most issues for your event.

Event Sign-Up Sheet

Use this to help plan your event by bringing it to the weekly member meetings or the monthly Board Meeting to recruit helpers keeping track of who participated in the event for the Cloverdale Lions Event Report that is due at the completion of the event.

Cash Countdown Sheet

Use this to make easy work of counting the proceeds of your days fund-raising activities. This sheet makes order of chaos for your treasurer, and are very easy and quick to use. They also serve as a good record and cross-check for your final event report.

It is also very important to the Treasurer for reconciliation, and sales and income tax purposes. We are a very busy club, so the treasurer puts in a lot of time for us. The value of this report and of the previous reports can not be stressed enough. It is also an extremely valuable tool for future Event Chairmen and our club to determine event success and the viability of individual fund-raisers. It is easier than any other alternative, including no report at all. It is also a requirement for all event chairmen.

Prospective Member Interview

This is to be used by Membership Retention Committee or the Executive Board as a tool for interviewing prospective members and keeping records of these interviews.
